To understand tinnitus, it helps to first understand how we hear. Sound travels from the outer ear through the ear canal to the eardrum, which vibrates and passes those vibrations through the middle ear bones (the malleus, incus, and stapes). These vibrations then reach the inner ear, where they are converted into electrical signals. These signals are sent through the auditory nerve to the brain, where they are interpreted as sound.
All this to say, we hear with our brain; the ears are simply messengers that deliver sound information. When everything functions properly, this system sends clear messages from the ear to the brain, but when parts of that system weaken, the message becomes distorted.
